Thrive Revival DNP Nursing Inc., doing business as Thrive Revival, is a California Nurse Practitioner Professional Corporation owned and operated by Dr. Ammie Kahl, DNP, FNP-BC, a California-licensed 104 Nurse Practitioner. We provide 100 % remote concierge primary care and functional medicine services on a cash-pay basis.

By using our website, scheduling appointments, or becoming a member, you agree to these Terms.

1. Services Provided

All care is delivered remotely via secure video or telephone.

Dr. Ammie Kahl, DNP, FNP-BC, is a board-certified, doctorally prepared Family Nurse Practitioner. She is not a physician and does not provide emergency or inpatient care.

2. Membership & Pricing (Cash-Pay Only)

- Annual Membership (primary member): $2,200 per year (paid upfront)

- Monthly Membership (primary member): $275 per month (auto-renews)

- Family Pricing: Additional household members (spouse or child under 27): $1,650/year or $206.25/month each

Membership includes:

• One 60-minute comprehensive 98+ advanced biomarker panel review per year and individualized care plan

• Follow-up labs with interpretation and an ongoing care plan

• Preventive care, acute/sick visits, and chronic-disease management

• Medication prescribing, refills, and ongoing adjustments

• Secure patient-portal messaging (response within 24 hours)

• Same-day telephone callback for urgent medical concerns

•Coordination of care, specialist referrals, forms, and superbills (HSA/FSA eligible)

Scheduled phone or video appointments: $100 each (billed immediately after)

Lab-Only Review (non-member): $500 one-time (includes panel + one 60-minute review)

Convert to membership within 30 days → full $500 credited

Important (applies to Membership and Lab-Only Review):

Advanced labs, imaging, and specialty screenings are ordered through third-party labs and radiology centers and billed separately. Patients may use insurance, where accepted, or pay cash directly to the lab and radiology centers. These costs are not included in membership or the lab-only service.

3. Payment & Billing

All fees are paid in advance through our secure portal (Elation Health).

We do not participate with insurance. Superbills are provided upon request.

4. Cancellation & Refunds

• Monthly membership: 30 days written notice required. No partial-month refunds.

• Annual membership: Non-refundable for first 90 days. After 90 days, a pro-rated refund of unused months minus a $450 administrative/close-out fee.

5. Remote Care Acknowledgment

You confirm you are physically located in California (or a state where Dr. Ammie Kahl, DNP, FNP-BC is licensed) during all visits and understand the benefits and limitations of remote care (BPC §2290.5).

6. No Guarantee of Results

Healthcare outcomes vary. No specific results are guaranteed.

7. Governing Law

These Terms are governed by California law. Venue: Ventura County.

8. Changes to Terms

We may update these Terms. Continued use of services constitutes acceptance.
