We offer 100% remote concierge primary care and functional medicine services on a cash-only basis (no insurance billing).   • Schedule appointments (via Calendly or Elation Health)

  • Complete intake forms or communicate with us

  • Pay for services (via Elation Health Billing – powered by Stripe)

  • Request superbills for visits (for potential insurance reimbursement at your discretion)

Device, browser, and usage data when you visit our website (via Squarespace analytics).

2. How We Use Your Information

  • To provide and manage your remote healthcare services:

  • To order and review labs through our partners (Quest Diagnostics, LabCorp, Genova Diagnostics, GI-MAP, MicroGenDX, SimonMed, Prenuvo, Galleri, etc.)

  • To process payments securely via Elation Health Billing

  • To communicate with you regarding appointments, results, and care plans

  • To improve our services and comply with legal requirements.

3. How We Share Your Information

We do not sell your information. We share it only when necessary:

  • With Elation Health (our HIPAA-compliant EHR and billing platform) – Business Associate Agreement in place

  • With diagnostic labs (Quest, LabCorp, Genova, etc.) for testing

  • With referring providers or specialists (with your written consent)

  • When required by law (e.g., public health reporting, court order)

4. Security of Your Information

All protected health information is stored and transmitted through Elation Health, a HIPAA-compliant platform. Payments are securely processed via Elation Billing (Stripe Connect). We maintain administrative, technical, and physical safeguards consistent with HIPAA and the California Confidentiality of Medical Information Act (CMIA).

5. Your California Privacy Rights

Under CMIA (§56 et seq.) and CCPA/CPRA, you have the right to:

  • Request access, correction, or deletion of your medical information
